Some forums said that we don't need to select that high of sample rate for gaming, because the sound card would then have to upsample the original audio source, potentially causing some lag in sound output. I never crank the master volume control all the way up for music though. Or maybe like what you said, the music have to be cranked all the way up. Change it to more than 2 channels, and the VU meter animation will start moving. I found that it doesn't work with 2 channels setting. By the way, what you said about 'audio levels not showing in mixers', I'm guessing you're talking about the VU meter animation.

Windows settings: In control panel>sound>speakers>configure, I just go 5.1 or 7.1 channels. There's a lot of posts that opposed the idea of using the front panel audio jacks at all (because of the low quality wires, noises, and stuffs), but I haven't pick up any discernible noises (beyond tolerance), and I'm not using my headset to listen to musics anyway. This way I can switch back and forth from using speakers and headset without plugging-unplugging any audio jacks.

I plug my headset in the front panel (front panel stereo + mic). It's a simple set, just 3.1 (2 front pair, 1 center, and 1 sub). Hardware: I have a set of speakers connected to the back panel. I do play left 4 dead 2, though I'm not sure the two of them behave the same in term of audio. I'm not much of a gamer, and I don't have CSGO. Like you, I've tried surfing the net, and tweaking the audio center back and forth to find 'optimized' settings. I'm using xonar DG to replace my onboard audio that died recently (about a week ago).
